A Jyotirlinga or Jyotirling or Jyotirlingam is a devotional Ling (mark) representing the god Shiva. The word jyotirlinga made by two words, Jyoti means 'radiance or shine' and lingam the 'mark or sign' of Shiva.
Jyotirlingam, means the Radiant sign of The Almighty power. There are twelve traditional Jyotirlinga temples in India.
This hymn (stotra) is recite or chanting to worship of those 12 jyotirlinga of lord shiva; The Dwadash Jyotirlingam Stotra is as follows –

Somnaath Sourashtra Me, Kashi Me Vishvesha |
Mahakaal Ujjain Me, Shivalaya Ghushmesh ||
Bheemshankar Dakini, Setubandha Ramesh |
Tryambakam Gomati Teer Par, Darukvan Nagesh ||
Mallikarjuna Shrishail Par, Himgiri Par Kedaar |
Chita Bhoomi Sthan Me VaidhyaNaath Bhavahaar ||
Omkar Mamaleshvar Me, Reva Tat Doye Naam |
Dwadash Jyotirlinga Ko Sada Karun Pranam ||
Sriddha Sahit Jo Dyavahi, Nis Din Me Do Baar |

It is believed that a person will see these lingas with devotion mind and attention, and then he reaches a higher level of spiritual attainment. And also get grace of lord Shiva.
How To Chant – It has to be recited in front of Lord Shiva with holy mind; you can chant this hymn twice in a day.
